NO CLINICAL INVOLVEMENT
*Must have current Driver's License & Auto Insurance*
- Common diagnosis / Types of patients/patient demographics: Home Health and Hospice Care. Adults, Geriatrics, Peds
- EMR HomeCare HomeBase
- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): 8a-5p. On call rotation for evenings/weekends
- Weekend Requirements: On call rotation
- Vacation Requirements: Vacation coverage required
- Job Responsibilities:
Initial assessment of client and family to determine home health needs - complete physical assessment & history of current & previous illness.
Regularly re-evaluates client nursing needs. Uses health assessment data to determine nursing diagnosis.
Initiates plan of care, makes necessary revisions as client status & needs change.
