Liquid Personnel is recruiting a Adult Social Worker to join its client’s Mental Health team, based in Calderdale.
What will your responsibilities be?
Carry out timely mental health assessments, including risk assessments, safeguarding enquiries, and strengths‑based care planning in line with the Mental Health Act and Care Act.Manage a complex caseload, providing ongoing support, crisis intervention, and coordinated multi‑agency work to promote recovery, safety, and wellbeing.Work collaboratively with clinical teams, housing, voluntary services, and families to ensure individuals receive holistic, person‑centred support and smooth transitions between services.
