Liquid Personnel is recruiting an Adults’ Social Worker to join its client’s Responsive Integrated Assessment Care Team (RIACT) based in Darlington.
What will your responsibilities be?
In this role, you will,
Undertake urgent assessments of adults in crisis to determine immediate care and support needs, aiming to prevent unnecessary hospital admissions.Work collaboratively with health professionals, reablement teams, and community services to implement short-term interventions that promote recovery and independence.Develop and review care plans within tight timeframes, ensuring safeguarding protocols are followed and outcomes are clearly documented.
