Saint Mary’s Hospital is looking for Per Diem LCSW/LMSW Social Worker to join our mission driven Case Management team. 

The Medical Social Worker is responsible for assessing and providing direct Social Work services to patients/families throughout the hospital setting upon admission and throughout discharge.

You Will Be Responsible For:

• Provide ongoing case management support to the patients.
• Assesses patient's needs and develops discharge care plans.
• Communicate effectively with staff, colleagues, patients, and family members. 
• Utilizes standard Social Work intervention techniques collaborating with patients and families.
• Work as a resource to other staff members.
