Job Description
Responsibilities - Conduct accurate and holistic speech and language specific assessment for clients during admission, periodic care plan reviews and discharge
- Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
- Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
- Design plans and conduct speech and language interventions for clients in a person centered manner, that is also evidence based
- Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
- Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
