Role Overview

The Community Rehabilitation Team will support people to reach an optimum level of function, focusing on activities of daily living, vocation and leisure. The post holder will work toward these goals by providing care for community patients and those planning discharge from the Rehabilitation Inpatient Units with severe mental illness and multiple complex needs, under the direction of qualified practitioners.

Key responsibilities include:

• Providing intensive support to people requiring a flexible recovery approach while avoiding fostering dependence.
• Working with trained staff to identify activities that the patient enjoys, finds meaningful and that occupy and divert them positively.
• Ensuring consistently high standards of record/case-note recording, maintaining electronic and integrated records in accordance with standards policy and relaying verbal information where appropriate.
