Job Description Job Summary:
Provide outpatient rehabilitative services to diverse patient populations, including pediatric, adult, and geriatric clients, to improve functional mobility and reduce pain. Deliver high-quality, evidence-based care in a fast-paced clinical setting.
Responsibilities:
- Assess patient condition through comprehensive evaluations and develop individualized treatment plans.
- Implement therapeutic interventions such as gross motor assessments, orthopaedic and neuro therapies, and post-operative rehabilitation.
- Monitor patient progress and adjust treatments to achieve optimal functional outcomes.
- Educate patients and caregivers on injury prevention, home exercise programs, and maintenance strategies.
- Document all clinical activities accurately within the electronic health records system.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity.
