Title: Physical Therapist (PT) Home Health
Location: Port Charlotte, FL 33948 (Coverage area: Port Charlotte, Englewood, Venice, Nokomis, Osprey, Sarasota up to 60-mile radius from hub)
Duration: 13 weeks
Shift: Day 5x8-hour shifts (8:00 AM 5:00 PM)
Job Responsibilities:
- Provide in-home physical therapy services to geriatric patients.
- Manage an average of 5 a7 patients per day.
- Perform evaluations, develop care plans, and deliver skilled PT interventions.
- Complete OASIS documentation (required for clinicians performing SOCs).
- Document patient care in HCHB EMR (required).
- Travel throughout assigned territory (up to 60-mile radius).
