Job Description

Title: Physical Therapist (Home Health) 

Location: Tulsa, OK

Duration: 13 weeks

Shift: Day, 8:00 AM – 5:00 PM, 5*8, 40 hours

Start date: 02/09/2026



Requirements:

  • Experience: 1+ year of Physical Therapy experience in Home Health with OASIS (SOC)

  • Certification: Active Oklahoma PT License, BLS (AHA)

  • Equipment: iPad (provided)

  • Charting System: HCHB (Homecare Homebase)

  • Travel experience required: Yes (no first-time travelers)

  • Patient Age Group: Geriatrics

  • On-call requirement: On-call rotation may be required
