**Job Description**
Working in close collaboration with a diverse and high functioning patient care delivery team, the Virtual Care Nurse is a central resource in caring for patients remotely. The overreaching goal of the Virtual Care Nurse is being responsible and accountable for coordinating timely, safe, effective care to patients remotely using a high level of technical and organizational skills as well as critical thinking ability.
The Virtual Care Nurse will be responsible for the coordination of all patient care activities remotely, including screening, triage, coordination of a multi-disciplinary team for safe, timely and high-quality care. This responsibility requires the Virtual Care Nurse to be a proficient and experienced clinician possessing an in-depth knowledge in all aspects of nursing care.
