The Case Manager will manage cases for their assigned clients to ensure that each client receives services needed for overall wellbeing and in accordance with the client's Individualized Treatment Plan (ITP). RESPONSIBILITIES: Utilize basic clinical interventions to support client efforts toward sobriety, coping with symptoms of depression and anxiety, safe lifestyle, healthy socialization, suicide prevention, occupational and academic success, individual and family restoration, and overall wellbeing. Acquire knowledge and maintain current contact and referral information of community resources to address client needs for support of sobriety and mental stability, food, clothing, housing, financial assistance, inpatient referrals, medical care, and safety.
