Ward 24 Health of older People servicesTe Whatu Ora Counties Manukau District is a progressive organisation at the forefront of innovative health delivery, working with New Zealand's most culturally diverse populations.Adult Rehabilitation and Health of Older People (ARHOP) provides specialist inpatient, outpatient services rehabilitation and community based services, the demands forwhich will continue in the future.Ward 24 is a 24 bed unit that focuses on assessment and treatment of elderly patients predominantly over 65 from the community, EC or acute services who require inpatient assessment, treatment and rehabilitation.Ward 24 utilises a multidisciplinary approach to bestow a holistic attitude towards the care and restoration of our elderly patients.Through consultation and discussion with the wider Multidisciplinary team collectively, we have developed models of care and service deliveries that are pertinent to the care and co-ordination our patients in ward 24 that we provide a service to.We are seeking a motivated nurse leader with sub-acute medical and rehabilitation experience to support and workin partnership with the Charge Nurse to lead our nursing team in Ward 24.You will enjoy the challenges that leading a team brings, by demonstrating excellence in clinical practice, management and team collaboration, providing high quality nursing care and commitment to patient focused outcomes.You will have a passion towards the care of our younger and older adults requiring rehabilitation.This role requires supporting and assisting the Charge Nurse with day to day coordination and management of the ward, working collaboratively with the Service Manager, Clinical Nurse Director, Nurse Educator, Medical and Allied Health teams.You will have a can do attitude, with the ability to be flexible, adaptable and meet deadlines.Essential Requirements :New Zealand Registered Nurse with a current annual Practicing CertificateMinimum of 5 years post registration with demonstrated leadershipPost GraduateDiploma relevant to Gerontology, RehabilitationHave an expert or proficient PDRP portfolioDemonstrated expertise in setting standards and managing practice to achieve standardsExperience inMedicine, Health of Older People and Adult Rehabilitation NursingExperience in staff managementPassion for working withadults on a rehabilitation journey#J-18808-Ljbffr
