Fracture Liaison services (FLS) are designed to reduce the incidence of future injury by offering appropriate assessment, education and interventions to people over the age of 50 years who experience low impact fragility fractures. Interventions are tailored to individuals and may include medications as well as information about osteoporosis and lifestyle measures that can assist bone health and reduce risk of future fractures.
We have commenced implementation of a Fracture Liaison Service in the Hutt Valley/Te Awakairangi and are now looking forward to extending the team across the Wellington, Porirua and Kapiti areas.
The Fracture Liaison team is small and friendly with two prescribing Coordinators, a Pharmacist and Nurse Practitioner, together with a non-prescribing CNS. The team also includes two administrators and a .2 fte Geriatrician in the Clinical Leader role.
Our Pharmacist Coordinator is looking forward to taking parental leave from September and we need someone to support our new service whilst she is away.
This position would be well suited to a Nurse Practitioner, Medical professional or Pharmacist who has an interest, and is able to prescribe in the area of bone health. You will also need a strong commitment to the health and well-being of Older People.
In this Coordinator role, you will be assisting with identification of people for follow-up, completing bone health and falls risk assessments, offering preventative education and, where indicated, initiating treatment before transferring to Primary care for ongoing oversight.
In the future, the District Community Older Persons services will work increasingly closely with Primary Health and Community Providers to assist ongoing support of people with complex health needs and to promote prevention of deterioration in health conditions. Our FLS Coordinators will have a particular focus on integrating services and facilitating treatment that will help reduce fractures in our populations.
An interest in quality improvement and innovation will be critical as we are a new service and will need your support to develop Fracture Liaison into a team that optimizes outcomes for our clients using the combined input of primary and secondary health services.

We interface with Emergency Departments, Orthopaedics, Geriatrics, Endocrinology, Rheumatology and Radiology within the hospital, also taking fragility fracture referrals from Primary Health. The Coordinator in this role will receive a solid orientation and support from existing staff whilst the team as a whole can access day to day collegiality from the large District-wide Community Older Adults service.

As this is a Coordinator role we are looking for someone with great interpersonal and networking skills, who will be a supportive team player. The primary location for this role will depend upon applicants, however there may be the need to work flexible hours from time to time and the role may include some travel within the District.
